Submitting Safety Reports

Air Maestro® contains a safety report register, which allows your organisation to manage information relating to safety events. When a hazard or occurrence eventuates, you can create a report on the system, recording all the relevant details. The system uses a wizard to prompt you for the required information depending on the report type and contributing factors.

When you lodge the report, all relevant personnel are advised of the report by email, an alert on the system or SMS. Relevant personnel can then participate in an investigation forum and discuss the issues raised from the event until a resolution is reached. Historical data is stored about each report for audit and reporting purposes. The safety report types are customisable for each organisation and vary according to you role and access levels. Report a deviation from your prescribed work practice controls. This report can only be submitted from the Timesheets screen, when you exceed a work practice control.

 Step-by-step guide:

  1. Select Safety → Safety Reports → Lodge New Safety Report. The new report screen is displayed.
    You can also create a report from the second alert screen. Click [Yes] when the system prompts you whether you have any safety reports to submit.

     

  2. Select the required report type from the Report Type drop-down list. The fields associated with the report type are displayed.
    The Reporter Details pane is automatically populated with your details.

     

  3. Enter the event details in the required fields. All required fields are marked with the asterix icon (*).
    The Confidential tick box enables you to hid the Reporter Details pane from all personnel who do not have access to view confidential reports. Please note that personnel with the View confidential reports access level are able to see you details, even you select this option.

  4. Click [Save] to proceed to the next tab in the report.
    If you have not entered data in a required field, the system will notify you, when you click [Save]. Complete the required field and click [Save] to continue.

  5. Complete all the relevant fields on the subsequent tabs and click [Next] to continue with the report.

    All mandatory tabs are indicated with the exclamation icon (). You must complete all the required fields on this tab before you can continue with the subsequent tab.

  6. Click the Review tab and review the report details.
    If you wish to modify the information, click the require d tab, modify the details and click the Review tab to submit the report.

  7. Click [Submit] on the Review tab to submit the report. The report is submitted for review. Alternatively, if you have access to submit to the authorities for the selected report type, the Submit to Authority dialog box is displayed. Select the required authority (if applicable) indicated by the relevant tick box and click [Submit]. If you select an authority in this dialog box, the report is sent to your internal safety manager, in addition to the external authority. This option is only enabled for certain personnel and you may not have the ability to perform this action depending on your access levels and Air Maestro® setup.
    To submit the report at a later stage, click any menu item to exit the Review tab. Each screen of the report is saved when you click [Next], so all the report information is retained until you are ready to submit the report.
